Baseline Questionnaire Past-Year Total Physical Activity Questionnaire (PYTPAQ) The PYTPAQ was designed to obtain information about the type and amount of physical activity undertaken by participants in the past twelve months, and was typically completed at the same time as the CDHQ-I. Follow-up Questionnaire Survey 2004 Survey 2004 was distributed to participants who joined ATP between 2000 and 2003. It contained questions on personal health history, cancer screening tests, sun exposure, smoking, body measurements, lifetime weight pattern, alcohol, sleep and shift work, health risk perception, quality of life, demographic characteristics and for women, hormone replacement therapy use. 404 2004

Follow-up Questionnaire Survey 2008 Survey 2008 was distributed in 2008 to research participants who joined ATP between 2001 and 2007. This questionnaire included information on lifetime residential history, personal and family health history, cancer screening tests, smoking, quality of life, fruit and vegetable intake, physical activity, sleep and shift work, built environment, work history, body measurements and demographic information. 507 2008

Baseline Questionnaire Updated Health & Lifestyle Questionnaire (UHLQ) This questionnaire was based on ATP’s original Health and Lifestyle Questionnaire (HLQ) completed by all participants who joined ATP between 2001 and 2008. Questions covered topics such as personal and family health history, health check-ups, reproductive health, past-year medication use, alcohol, smoking, sun exposure, sleep, occupation and demographic characteristics. 302 2009-2011

Baseline Questionnaire Physical Activity & Nutrition Survey (PANS) This survey asked about physical activity and dietary habits. The physical activity section was the long form of the International Physical Activity Questionnaire, while the nutrition sections asked about consumption of servings of several food groups similar to those used in Canada’s Food Guide. 116 2009-2011

Baseline Questionnaire CORE This questionnaire replaced the UHLQ and PANS and was a refined version of them. Small changes were made based on feedback from research participants to make the questionnaire easier to complete. It included sections on demographic characteristics, cancer screening tests, reproductive health, personal medical history, current medication use, family health history, sleep, sun exposure, food consumed, alcohol, smoking, physical activity, work and body measurements. It is recommended that if you request data from CORE, that you also choose the similar variables from UHLQ/PANS. 423 2011-2015

Follow-up Questionnaire Survey 2017 Survey 2017 was a follow-up questionnaire distributed to ATP participants between 2017-2019.The questionnaire collected updated information on participants’ health, including cancers and long-term health conditions, cancer screening, mental health, quality of life, alcohol consumption, smoking status, body measurements, as well as family health history. Follow-up Questionnaire COVID-19 This questionnaire was designed to collect information about the impacts of COVID-19 on Albertans. It asked participants about COVID-19 test results, suspected infection, symptoms experienced (if any), hospitalization, current health status and risk factors, potential sources of exposure and the impact of the pandemic on job status, and well- being (mental, emotional, social and financial). Invitations were sent from June to July 2020 and the questionnaire closed in September 2020. 632 2020

Follow-up Questionnaire COVID-19 Antibody Pre-appointment Questionnaire Time 0 The COVID-19 Antibody Testing (CAT) study collected samples and data on 4000 ATP participants. Participants were invited to complete surveys and attend a study center to provide blood samples at four different time-points (0, 4, 8, and 12 months). The CAT baseline questionnaire collected data during the first time-point of the CAT study which was completed in Calgary (Sept/20-Jan/21), Red Deer (Dec/20-March/21, Edmonton (Jan/21-Apr/21), and Lethbridge (Feb/21-May/21). Other Datasets Biomarkers Blood sample collection for CanPath began December 19, 2008 and ended on July 31, 2015. Approximately 50 mL of non-fasting blood was collected from each of 29,183 consenting research participants who attended a study centre. Post collection, blood samples were divided into 0.5mL or 1mL cryovial aliquots.
